1 hour Treatment session $150 (prompt pay cash rate)

In home 1 hour Treatment $175 (cash rate)

Package of 10 Treatment sessions $1350

Dry Needling (when not part of session) $50 for 30 minutes

Wellness Assessment $150

Please have your physician fax a referral for physical therapy to 847-745-0103.

We are currently in network with Blue Cross Blue Shield, please check with your provider for coverage and benefits. All other insurance is out of network, service can be provided at cash rate with Superbill provided to allow you to submit to your insurance company for reimbursement, please check your out of network benefits.
